Investigation into a fatal accident at Fairfield level crossing, Bedwyn, Wiltshire on 6 May 2009

The RAIB is carrying out an investigation into a fatal accident at Fairfield level crossing, Bedwyn, Wiltshire on 6 May 2009.

The accident occurred at 17:30 hrs when the 17:08 hrs First Great Western service from Newbury to Bedwyn struck and killed a woman and a dog on the level crossing.

The RAIB’s preliminary examination indicates no issues with the condition or operation of the signalling system, or the condition or operation of the train that could have contributed to the accident.

The RAIB’s investigation is independent of any investigations by the safety authority.

The RAIB will publish a report, including any recommendations to improve safety, at the conclusion of its investigation. This report will be available on the RAIB website.
