Investigation into a fatal level crossing accident at Staines, Surrey on 16 April 2008

The RAIB is carrying out an investigation into a fatal level crossing accident at Moor Lane footpath crossing, Staines, on 16 April 2008.

The accident occurred at 08:13 hrs when the 07:28 hrs South West Trains Service from London Waterloo to Windsor struck and killed a 66 year old woman on the level crossing, which carries a public footpath over Network Rail’s line from Staines to Windsor.

The RAIB’s preliminary examination indicates no issues with the condition or operation of the train or the signalling system that could have contributed to the accident.

The RAIB's investigation is independent of any investigations by the safety authority.

The RAIB will publish a report, including any recommendations to improve safety, at the conclusion of its investigation.  This report will be available on the RAIB website.
